I have just installed PM Gold on my MacBook Pro OS X 10.7.5. Trying to print original note card using Avery 8315 template. Preview shows it will print my first page four times, rather than front and back page (blank) twice. What am I doing wrong?

OK, on my PC, I got the exact same thing when creating a Avery 8315 note card from scratch. So this project is buggy. I developed a work-around. Do a new quarter-fold card. You will see it has 4 "pages". Design the front of the card on page 1. Select all, group and copy page 1 onto page 3. Print. This will give you two note cards on a sheet with blank insides and back.
