This is something I do so infrequently that I am no expert about it. However, I do remember how important it was to provide a CSV that matched the headers that Print Shop uses. Open your CSV file and edit headers accordingly.
For example, I created a New Address Book in Print Shop (with just one name as a test) then exported it as CSV and this is what I got:

So if you were to edit a copy of your flat file to reflect this (you can delete columns that you don't use), I believe your edited file will import fine.
