David Using PS 3.5 the city listed in the city field of your address book is "primary city" in mail merge. I don't know why they did that and didn't make things match, thereby confusing the rest of us. You'll also find that if you want to use the title field from the address book, you'll find that listed as "prefix" and not salutation or title. Hope this helped!
