Just create a project from scratch, choose labels as the project type, and then select custom. You can define the size you want for your label.

You would use width: 4.0 and height: 3.333. I'm guessing the gaps in between the labels are 0.0 as I don't have a label sheet to look at.

There is no import function for templates. This easy Custom Size template maker takes care of most label sizes. Only label sizes with a height less than one inch won't work.

Did you complete the process?

File> New> Create from Scratch.
Select Labels from the Project Type drop-down.
Fill in the data fields as shown. Click OK. This creates your new label project which is now ready for you to add graphics and text:

After you add your graphics and text, you are ready to print. The program automatically duplicates your design onto all six labels. Here is my Print Preview:

Save the project. Whenever you want to use Avery 5164, you can open this project, make your edits, then save as a different name.

Edit> Convert to Full Avery Sheet.... This allows you to individually edit each label so that they don't all have to be exactly the same, or so you can print select labels and not all six.
