The title field is missing, but can be replaced by the Prefix field. Choose Prefix from the box containing the mail merge fields, then click Insert field.

OLD SOLUTION
The title field is missing. A workaround is to add your own Custom field as shown in the screen captures below.
In the Address Book, click the Custom Tab. Add the contact's title in the first Value box.
In your Label Project, insert mail merge fields from the drop down box. Custom 1 Value is now your contact's Title.
